Join us for Live Heart Smart, a half-day event dedicated to heart health. This fun and interactive educational event will leave you with plenty of practical knowledge and strategies to begin or recharge a Heart Smart way of life!
Live Heart Smart will include:
• Welcome/ North Country Community Health Improvement Plan (CHIP) Overview
• Keynote address: "The Heart is the Seat of the Body"
Breakout Session One:
• "Living an Anti-Inflammatory Life" - Learn the role of inflammation in the body, it's connection to heart disease, and how to control inflammation through lifestyle factors, including diet and nutrition.
• "Practical Fitness for Heart Health" - Understand and apply simple and effective "do anywhere" fitness strategies to begin and progress on a program for heart health and overall wellness, regardless of your age or current ability level.
• "Community Health Workers" - Learn about the role of community health workers (CHW) in the health care landscape; discover how CHWs can help patients manage chronic conditions and navigate a heart-healthy plan. Breakout sessions
Breakout Session Two:
• "Take a Moment for Yourself: Mindfulness Tools to Reduce Stress and Encourage Heart Health" - Understand the concepts of mindfulness and how it can support heart health; Experience mindfulness through guided practice and discover ways to incorporate mindfulness into daily life.
• "Move Your Cheese" - Identify root causes and lifestyle behaviors that increase the risk of heart disease; explore how ambivalence impacts health behavior and develop a mental tool box to improve your consistency to adopt behavior change.
