AUDIENCE: Community member leaders who manage their own and/or are caregivers who helps others to manage their chronic condition(s), and have the desire to lead others in the community to thrive with their chronic condition(s).
Leaders who have been previously trained in CDSMP and are currently active are eligible to participate.
Do you manage a chronic condition and/or are a caregiver for someone who does? Join the upcoming Chronic Disease Self-Management New Leader Training! You do not have to be a health professional to participate. This 4-day training will prepare you to lead the 6-week CDSMP workshop series known as Better Choices, Better Health, an evidence-based program of Stanford University. These workshops are held in the community and lead by trained CDSMP trainers. For this New Leader Training, it is beneficial to partner with an already-trained CDSMP leader, or attend the Training with a partner, if possible, as you will be asked to commit to offering at least one 6-week workshop with a co-leader within one year, and ideally 2 – 3 trainings per year, to sustain the program.
