The following Community Health Worker (CHW) position is a full-time (40 hrs.) opportunity with the Ways2Wellness Connect program and reports to the W2W Program Coordinator/Supervisor.
JOB DUTIES
• Provide outreach, community education, social support, and advocacy as the liaison between health care providers and the patient • Assist patients with removing barriers to accessing health care and other services
• Work in a team-based environment and with NCHC staff • Collect and report program and patient data to care teams
QUALIFICATIONS/ REQUIREMENTS
- Must have resided within the local community for 2+ years with knowledge of local culture and values, and familiarity with the resources available within the community
- Willingness to travel within northern NH
- Experience and comfort with Microsoft Office Suite, especially email, calendar scheduling, PowerPoint, and Exce
- Valid driver’s license, reliable transportation, and proof of insurance
- Willingness to complete CHW training (provided by NCHC)
- Bachelor’s Degree or High School Diploma/GED and comparable experience
