Job Description

Region 7 Integrated Delivery Network (IDN) is a statewide program to improve the

health outcomes for Medicaid participants with mental health conditions, substance use disorders, or substance misuse through a coordinated network of behavioral health, medical, and social service providers. North Country Health Consortium is looking for an organized, collaborative, and flexible team member to engage with Region 7 IDN members in Coos, Carroll and northern Grafton Counties to build regional capacity to deliver integrated physical and behavioral health care. Responsibilities will include coordinating and implementing various projects to meet the goals and objectives of the IDN. Additional responsibilities will include scheduling and facilitating meetings, and website maintenance. This is a full-time position, based in Littleton, NH.

Position requires strong written and verbal communication skills. Candidates must be highly organized and comfortable working with the public and with a team. Reliable transportation is needed to travel within Coos, Carroll and northern Grafton Counties.

Qualifications for IDN Project Coordinator:

Bachelor’s degree in healthcare, or related area preferred. Experience with project coordination, data reporting, website maintenance, MS Office Suite, and project reporting preferred.
