Community Health Worker/Recovery Coach (CHW/RC)

The following Full-Time position is available within NCHC’s Wellness And Recovery Model (WARM) Program. WARM works to integrate recovery supports, providing care coordination and resource navigation, to enhance access to treatment and recovery for substance use disorder (SUD), and improve the knowledge and understanding of community members and professionals regarding best-practices around SUD.


• Assume role of liaison between WARM clients and treatment providers, offering continuous communication and monitoring, education, social support and service coordination for clients
• Work in a team-based environment with NCHC staff • Provide status reports for clients
Collect and report program and client data to care teams


  • Bachelor’s Degree or High School Diploma/GED and comparable experience
  • Residence within the local community for 2+ years with knowledge of local culture and values, and familiarity with the resources available within the community
  • Willingness to travel within northern NH
  • Comfort with Microsoft Office suite, especially email and calendar scheduling
  • Valid driver’s license, reliable transportation, and proof of insurance
  • Willingness to complete CHW or RC training (provided by NCHC)
