Job Description

The following full-time position is available in the North Country Opioid Use Disorder Outreach Program. This program will implement a “warm handoff” model in Emergency Departments for patients presenting with an opioid use disorder, enhance care coordination practices by integrating community health workers/recovery coaches into treatment and referral protocols, and improve knowledge and understanding by law enforcement regarding the best practices for identifying, screening, and linking opioid abusers to treatment.

Job duties:

  • In a team-based approach: oversee the coordination, professional development, and deployment of the Community Health Workers/Recovery Coaches •responsible for collaborating with the Program Director, Advisory Group members, Federally Qualified Health Centers, Critical Access Hospitals, Social Service Agencies, and other pertinent care coordination teams and staff treatment providers, and recovery resources to recognize the unique health needs for patients referred to the program.

Qualifications/Requirements:

  • Bachelor’s Degree in a related field • Ability to work with and manage multi-disciplinary team • Strong organizational, interpersonal, writing and communication skills• Ability to set up and maintain efficient and effective systems and processes to manage details and grant deliverables • Willingness to travel within northern NH
