Gain the skills & core competencies to work as a Community Health Worker (CHW)— or in other roles under the CHW umbrella including, Patient Navigator, Health Assister, Outreach Worker, Home Visitor, Patient Adviser, Patient Advocate etc.— in this 9-week class series! A frontline public health worker who is a trusted member of the community with a close understanding of the North Country, CHW's serve as a liaison between the patient and healthcare and social services to facilitate access to care and improve the quality and cultural competence of service delivery. Must have a high school diploma and access to a computer/internet. Course topics include: • Communication Strategies • Care Connections • CHW Processes • Health-related topics • Motivational Interviewing (2-day class) • Resource Building • Problem Solving • Action Planning.
